About the Role

We are seeking a highly skilled and compassionate Nurse Practitioner to join our Advanced Illness Management team at Illumia. As a key member of our clinical team, you will play a vital role in delivering high-quality, patient-centered care to individuals with complex medical needs.

Key Responsibilities
  • Provide comprehensive assessments and develop individualized treatment plans for patients with advanced illnesses
  • Collaborate with interdisciplinary teams to ensure seamless care coordination and communication
  • Prescribe medications and order diagnostic tests as needed, while maintaining accurate and up-to-date medical records
  • Participate in ongoing education and professional development to stay current with best practices in palliative care
  • Contribute to quality improvement initiatives and participate in peer review processes
Requirements
  • Master's degree in Nursing from an accredited institution
  • Current licensure as a Registered Nurse and Nurse Practitioner in the state of practice
  • Minimum of two years of experience in palliative care or a related field
  • Strong leadership and communication skills, with the ability to work effectively in a fast-paced environment
  • Knowledge of palliative care principles, including symptom management and end-of-life care
