Home Health Nurse Case Manager Position

Job DescriptionWe are seeking a highly skilled and compassionate Registered Nurse (RN) to join our team as a case manager in homecare. As a case manager, you will be responsible for performing patient assessments, developing and implementing plans of care, and collaborating with the care team to ensure seamless patient outcomes.
- Perform patient assessments and develop individualized plans of care to address patient needs and goals.
- Collaborate with the care team, including physicians, therapists, and other healthcare professionals, to ensure comprehensive patient care.
- Supervise Licensed Practical Nurses (LPNs) and Home Health Aides (HHAs) to ensure quality patient care and adherence to policies and procedures.
Our ideal candidate will possess excellent communication and interpersonal skills, with the ability to work effectively in a fast-paced environment. The successful candidate will also have:
- One (1+) year of clinical experience as a Registered Nurse (RN). If less than 1 year clinical experience as a RN, candidate must be approved by VP Clinical.*
- Current RN license, specific to the state(s) you are assigned to work.
- Current CPR certification.
- Valid driver's license, reliable transportation, and liability insurance.
We offer an attractive salary range of $74,880K - $96,720K, depending on location, job-related knowledge, skills, and experience. Our compensation reflects the cost of labor across several U.S. geographic markets. In addition to a competitive salary, we offer a comprehensive benefits package, including:
