Overview of the Position

We are seeking a compassionate and skilled Registered Nurse or Licensed Practical Nurse to join our team as a Clinical Care Coordinator in our Long-Term Care community. Your Responsibilities

As a Clinical Care Coordinator, your primary responsibilities will include:

  • Providing high-quality clinical care to our residents with compassion and empathy
  • Collaborating with our interdisciplinary team to develop and implement individualized care plans
  • Ensuring seamless communication among team members and stakeholders to achieve optimal patient outcomes
  • Maintaining accurate and up-to-date records of resident care and progress
