Key Responsibilities:- Perform comprehensive assessments and develop individualized care plans to meet the unique needs of each patient.
- Collaborate with interdisciplinary teams, including discharge planners and social service workers, to ensure seamless care transitions.
- Conduct regular home visits to evaluate the quality of care and provide education to patients and their families.
- Participate in ongoing assessment and review of patient needs, making adjustments to care plans as necessary.
- Develop and maintain strong relationships with patients, families, and healthcare providers to ensure comprehensive care.
- Graduate of an accredited school of nursing with a current license as a Registered Nurse in the State of MN.
- At least two years of current nursing experience, with a minimum of one year in case management.
- Strong communication and interpersonal skills, with the ability to work effectively in a fast-paced environment.
- Reliable transportation, valid driver's license, and proof of auto insurance.
- Community health experience and a wide range of medical-surgical experience.
- CPR certification.
- An annual salary range of $84,905 - $101,233.
- A $6,000 hiring bonus.
- A company car option or mileage reimbursement.

- 100% clinical license reimbursement.
- CEU reimbursement up to $400 per year and 2 days of CEU time off.
