Job Description

As a Registered Nurse Case Manager with Interim HealthCare of the Twin Cities, you will have the opportunity to work independently and autonomously in a home care setting, visiting patients one-on-one in their own homes. This role requires a strong nursing background and excellent communication skills to effectively assess and review patient needs, develop working relationships with discharge planners and social service workers, and participate in ongoing interdisciplinary assessment of the patient.

Key Responsibilities:

  • Perform assessments and procedures, which require knowledge, judgment, and nursing skill based upon principles of psychological, biological, physical and social sciences.
  • Develop working relationships with discharge planners and social service workers.
  • Assess and review the matching of employee skills to client needs.
  • Visits in home clients to evaluate the quality of care being provided and review clinical notes.
  • Participate in ongoing interdisciplinary assessment of the patient.

Requirements:

  • Graduate of an accredited school of nursing and has a current license as a Registered Nurse in the State of MN.
  • Able to travel within a geographic area serviced by the office. Reliable transportation, valid drivers license, and proof of auto insurance is required.
  • CPR Certification
  • Two (2) years of current nursing experience.
  • One (1) year case management experience preferred.
  • Preferred: Community health and a wide range of Medical-Surgical experience.
