Physical Therapist Home Care Specialist

As a Physical Therapist Home Care Specialist, you will play a vital role in our mission by providing personalized physical therapy services to patients in the comfort of their own homes.
Your primary responsibility will be to design and implement individually tailored physical therapy programs to maintain, improve, or restore physical functioning, alleviate pain, and prevent physical dysfunction in patients.
Key responsibilities include:
- Conducting initial exams to evaluate patient data and identify functional impairments, and managing caseloads by scheduling appropriate therapy revisits and reassessments as needed.
- Developing and implementing physical therapy treatment plans to maintain, improve, or restore physical function and alleviate pain.
- Reviewing physician referrals and patient medical records to determine diagnosis and required physical therapy treatment.
- Administering therapeutic exercises, range of motion treatment, gait training, stair training, home safety training, and therapeutic activities to improve function, increase endurance, and enhance patients' quality of life.
- Recommending appropriate equipment and home modifications to improve patients' function in the home environment and enhance caregivers' ability to assist patients.
- Assigning patients to physical therapy assistants as appropriate and providing supervision according to the PT practice Act and Penn Medicine policies.
- Discharging patients from physical therapy when goals or projected outcomes have been attained or patients are no longer homebound, and providing for appropriate follow-up care or referrals after discharge.
- Performing duties in accordance with Penn Medicine and entity values, policies, and procedures.
- Other duties as assigned to support the unit, department, entity, and health system organization.
