We are seeking a highly skilled Physical Therapist to join our Home Care team in Princeton North. As a member of our team, you will have the opportunity to work collaboratively with a multidisciplinary team to provide high-quality patient care and make a meaningful impact in the lives of our patients.

Key Responsibilities
  • Conduct thorough initial evaluations to identify functional impairments and develop personalized treatment plans to improve patient outcomes.
  • Collaborate with the interdisciplinary team to ensure seamless care coordination and effective communication.
  • Administer therapeutic exercises, range of motion treatment, and other interventions to improve patient function and alleviate pain.
  • Provide education and training to patients, families, and caregivers to promote independence and optimal outcomes.
  • Monitor patient progress, adjust treatment plans as needed, and communicate findings to the team and physicians.
  • Assign patients to physical therapy assistants as appropriate and provide supervision in accordance with PT practice act and PCAH policies.
  • Discharge patients from physical therapy when goals are met or patient is no longer homebound, and provide for appropriate follow-up care or referrals.
Requirements
  • Basic Cardiac Life Support certification
  • Valid driver's license
  • Physical Therapist licensure in Pennsylvania and/or New Jersey, depending on location and scope of responsibilities
  • Graduation from a CAPTE-accredited school of physical therapy or a nationally recognized organization by the Pennsylvania State Board of Physical Therapy
