Key Responsibilities- Assessment and Care Planning: Conduct thorough assessments of patients with wounds, ostomies, and other complex care needs. Develop and implement individualized care plans to ensure optimal outcomes.
- Education and Support: Provide education and support to patients, families, and healthcare teams on wound and ostomy care best practices.
- System Improvement: Identify opportunities for system-wide improvement in wound and ostomy care. Develop and implement evidence-based policies, procedures, and protocols to enhance patient care.
- Collaboration and Communication: Work collaboratively with interdisciplinary teams to coordinate care and ensure seamless transitions. Communicate effectively with patients, families, and healthcare teams to ensure informed decision-making.
- Leadership and Professional Development: Serve as a clinical preceptor for undergraduate nursing students and staff. Participate in professional development opportunities to stay current with best practices in wound and ostomy care.
- Education: Bachelor's Degree in Nursing
- Experience: 3 years of RN experience
- Certifications: RN - REGISTERED NURSE - NLRN, Certified by WOCNCB as a CWOCN, CWON, CWCN or COCN (Lawson Code COTR)
