Job Summary:

The Transitional Housing Homeless Outreach Worker will serve as the first point of contact for homeless youth aged 16 to 24, providing a non-judgmental and supportive approach to helping them overcome their crisis. Key responsibilities include:

1. Connecting youth with essential resources and services to address their immediate needs.

2. Providing case management and counseling to youth in crisis, empowering them to make informed decisions about their lives.

3. Conducting thorough assessments to determine eligibility for Catholic Charities' services, including the onsite transitional house.

4. Building and maintaining relationships with community resources to ensure a comprehensive support network for our clients.

Requirements:

• Bachelor's Degree in Social Work or a related human services field.

• 2 years of experience working with youth, preferably in homelessness, mental health, or chemical health.

• Must be at least 21 years old and possess a valid driver's license with a working vehicle.
