Position Overview:
Delta-T Group is currently seeking a dedicated Community Case Manager to deliver essential services to clients in the Hartford area. This role focuses on supporting veterans and individuals transitioning into stable housing.
Key Responsibilities:
- Collaborate closely with veterans to assess their needs and provide tailored social services support.
- Facilitate the identification and documentation required for securing vital resources such as Medicaid and housing subsidies.
- Assist clients with daily living tasks, including budgeting, shopping, cooking, and bill management.
- Promote social engagement through peer activities and community connections.
- Integrate mental health and addiction treatment with wellness services to enhance client well-being.
- Connect clients with job services and alternative employment opportunities.
- Maintain ongoing case management support for clients post-housing to ensure their continued success.
Qualifications:
- A Bachelor's Degree or equivalent experience is required.
- Bilingual proficiency in Spanish is preferred.
- Previous case management experience in supportive or transitional housing is advantageous.
Work Schedule:
This position offers full-time hours, Monday through Friday, from 8:30 AM to 4:30 PM.
