Job Title: Skill BuilderJob Summary:

Meridian Health Services is seeking a skilled and compassionate Skill Builder to join our team. As a key member of our healthcare team, you will provide skills and development training to assist patients and/or families in developing skills directed towards achieving developmental milestones and reducing psychosocial barriers.

Key Responsibilities:
  • Assist the Therapist in developing, implementing and revising individual treatment plans to ensure that services provided are specified in the treatment plan and monitor progress toward treatment goals.
  • Provide individual and group skills training and development to assist the patient and/or family in developing skills such as self-care, daily life management, interpersonal communication, parenting, illness management, anger management, positive coping skills, and problem solving skills.
  • Provide case management services as identified in the patient's treatment plan, working collaboratively with the Therapist and medical staff to ensure coordination of integrated care for the patient.
  • Complete CANS/ANSA re-assessments and provide consultation to staff on specific cases involving coordination of care.
  • Maintain positive working relationships with staff of other agencies and institutions, homes, and facilities, and act as a liaison between patients and agencies and/or institutions.
  • Prepare and maintain clinical records, reports and forms and other administrative tasks as required.
Requirements:
  • High school diploma or its equivalent with 2 years paid behavioral healthcare experience or equivalent college coursework toward a human services or health related degree required at minimum.
  • Associate or Bachelor's degree preferred.
  • A valid driver's license with a driving record meeting Meridian's driving policy, reliable transportation, and proof of auto insurance.
  • A commitment to providing individual attention to help clients achieve mental, emotional and physical well-being.
