Job Title: Healthcare Professional - Skill Builder

Meridian Health Services is seeking a skilled Healthcare Professional to join our team as a Skill Builder. This role will provide skills and development training to assist patients and/or families in developing skills directed towards achieving developmental milestones and reducing psychosocial barriers.

Key Responsibilities:

  • Assist the Therapist in developing, implementing and revising individual treatment plans.
  • Provide individual and group skills training and development to assist the patient and/or family in developing skills directed toward reducing psychosocial barriers.
  • Work collaboratively with the Therapist and medical staff to ensure coordination of integrated care for the patient.
  • Complete CANS/ANSA re-assessments.
  • Provide consultation to staff on specific cases involving coordination of care.

Requirements:

  • High school diploma or its equivalent with 2 years paid behavioral healthcare experience or equivalent college coursework toward a human services or health related degree required at minimum.
  • A valid driver's license with a driving record meeting Meridian's driving policy, reliable transportation, and proof of auto insurance.
  • A commitment to providing individual attention to help clients achieve mental, emotional and physical well-being.
