Clinical Care Coordinator Job Description

Catholic Health Services seeks a skilled Clinical Care Coordinator to provide high-quality care and support to patients and their families. As a Clinical Care Coordinator, you will play a crucial role in delivering comprehensive hospice services, ensuring patient-centered care, and promoting a collaborative team environment.

• Assessing patient needs and developing individualized care plans in collaboration with healthcare teams

• Coordinating and implementing care services, including medication management, home health visits, and other interventions as needed

• Educating patients and caregivers on disease management, symptom control, and end-of-life care

• Facilitating communication among healthcare providers, patients, and family members to ensure seamless care transitions

• Monitoring patient progress, identifying areas for improvement, and making adjustments to care plans as necessary

• Collaborating with interdisciplinary teams to develop and implement comprehensive care plans

Requirements:

To be successful in this role, you will need:

• A Bachelor's degree in Nursing (BSN) or a related field, along with current licensure as a Registered Nurse (RN) in the state of Florida

• At least 1-2 years of experience in nursing, preferably in a hospice or palliative care setting

• Strong knowledge of hospice policies, procedures, and regulations, including Medicare and Medicaid requirements

• Excellent communication, interpersonal, and organizational skills, with the ability to work effectively in a fast-paced environment

• Ability to lift and move patients according to accepted nursing standards of body mechanics, with occasional physical demands such as walking, climbing stairs, or standing for extended periods

Catholic Health Services offers a competitive salary range of $60,000 - $80,000 annually, depending on experience.
