At Chelmsford Crossings, we are seeking a dedicated LPN or RN to become an integral part of our community. As a Licensed Practical Nurse or Registered Nurse, your primary responsibility will be to provide exceptional nursing care to our residents in a nurturing and homely atmosphere.

You will collaborate with a team of passionate professionals who share your commitment to building meaningful relationships with both residents and colleagues.

Position Overview:

This is a full-time position with day shifts and a weekend rotation. If you are dedicated to making a difference in the lives of others and take pride in your nursing practice, we invite you to explore this opportunity.

Key Responsibilities:
  • Conduct thorough assessments, communicate effectively, and document residents' health status.
  • Monitor and report any significant changes in resident conditions to the appropriate personnel.
  • Supervise and guide the daily activities of Resident Care Assistants.
  • Mentor Lead Resident Care Assistants in their roles.
  • Coordinate care with community providers through an efficient case management approach.
  • Support resident independence while balancing service demands.
  • Recognize and assist in preventing elder abuse and neglect, reporting as required.
  • Respond to emergencies and utilize the personal emergency response system appropriately.

Qualifications:

To be considered for this role, candidates must possess:

  • A current state license as an LPN or RN and CPR certification.
  • A graduate of an accredited LPN or RN program.
  • A minimum of 1 to 2 years of nursing experience is preferred.
  • Experience working with the elderly in any capacity is highly desirable.
