Nurse Practitioner

The Registered Nurse provides skilled professional nursing care to patients, families, and caregivers as prescribed by physicians and in compliance with state regulations, patient needs, and Bluegrass Care Navigators policies.
Responsibilities- Delivers high-quality patient care, adhering to professional standards and principles.
- Collaborates with interdisciplinary teams to develop and implement patient care plans.
- Provides education and support to patients, families, and caregivers on disease management, treatment options, and end-of-life care.
- Conducts thorough assessments, identifies patient needs, and develops individualized care plans.
- Administers medications, performs treatments, and provides wound care as prescribed.
- Documents patient information, progress, and outcomes accurately and timely.
- Participates in quality improvement initiatives, patient satisfaction surveys, and peer review processes.
- Graduate of an accredited school of professional nursing.
- Current RN licensure through the State Board of Nursing.
- At least one year of general nursing experience, preferably in hospice, home health, medical, surgical, or critical care.
- Experience with pain management, IV skills, and end-of-life care is preferred.
- Excellent observation, communication, and documentation skills.
- Ability to work independently, with minimal supervision, and as part of an interdisciplinary team.
- Competitive salary range: $65,160-$82,536 per year.
