Pediatric Nurse Case Manager

The Kidz Club PPEC provides daily skilled nursing care for children, ages birth to under 21, who have been diagnosed with medical complexities. Our pediatric day-shift opportunity offers limited Saturdays, closed nights, Sundays, and major holidays. Compensation
The estimated annual salary for this position is $54,000 - $65,000 based on the title, job requirements, and location in Lexington, KY. The starting rate is $26.00 - $32.00 per hour, increasing with experience. A $2,000 sign-on bonus is also available for new full-time RNs.
Job Description
The Registered Nurse Case Manager (RNCM) manages the care of pediatric patients according to their individual needs and care plan using a multidisciplinary method. The RN facilitates referrals to primary providers, specialists, healthcare facilities, and community resources. Direct client care includes a holistic approach to the child's and family's educational, psychosocial, behavioral, physical, mental, and emotional needs.
Qualifications
To be eligible for this role, you must be a graduate of an accredited school of nursing with current RN licensure eligible in the state of Kentucky. Recent graduates with provisional licenses are also considered. You should have basic computer skills and comfort with electronic charting systems. At least six months of current experience as a practicing RN with pediatric experience is preferred.
