Key Responsibilities:
  • Assess patient needs and develop personalized care plans to ensure optimal health outcomes
  • Provide comprehensive nursing care to patients in their homes, including wound care, medication management, and education
  • Collaborate with interdisciplinary teams to coordinate patient care and ensure seamless transitions
  • Communicate effectively with patients, families, and healthcare providers to ensure informed decision-making
  • Stay up-to-date with the latest clinical guidelines and best practices in home health care

Requirements:
  • Current, unrestricted state RN license
  • 1+ year experience in a clinical care setting as a nurse
  • Able and willing to travel within branch/office coverage area
  • Must have a dependable vehicle, valid driver's license, and current auto insurance in accordance with state laws
  • Must be able and willing to travel 50%
