  • Fill in gaps in coverage on shifts, or act as the starting caregiver when onboarding new clients.
  • Develop and implement care plans, working closely with clients, families, and healthcare professionals to ensure continuity of care.
  • Train and support caregivers, ensuring they provide high-quality care and meet the needs of our clients.
  • Communicate effectively with clients, families, and healthcare professionals to ensure seamless care coordination.
  • Monitor client progress, identifying areas for improvement and implementing changes as needed.
  • Ensure compliance with healthcare regulations and agency policies, maintaining the highest standards of care and quality.
Requirements
  • Associate's degree or equivalent experience in a related field.
  • Minimum 2 years of caregiving experience, with a strong background in healthcare.
  • CNA or HCA license, with a valid Driver's License and willingness to travel to multiple clients in a day.
  • Nurse Delegation certificate, with excellent communication and problem-solving skills.
  • Ability to work independently, with a strong attention to detail and a commitment to delivering exceptional care.
