About the Role
We are seeking a compassionate and experienced Nurse Practitioner to join our team at Abode Care Partners.
Overview
As a Nurse Practitioner, you will provide evidence-based medical care to seniors and patients with complex needs. You will collaborate with a multidisciplinary team to evaluate patient conditions, perform diagnostic tests, and ensure comprehensive care while maintaining up-to-date medical records.
Responsibilities
- Provide primary and specialty healthcare services to patients in various settings, including skilled nursing facilities, assisted living, independent living, group homes, and private homes.
- Conduct physical exams, diagnose and treat acute and chronic illnesses, and order laboratory and diagnostic testing as needed.
- Collaborate with caregivers and other healthcare professionals to develop and implement individualized plans of care.
- Ensure accurate and timely documentation of patient medical records.
- Maintain current knowledge of best practices in nursing and adhere to established guidelines and protocols.
Requirements
- Active state Nurse Practitioner license.
- Current Nurse Practitioner National Board Certification.
- Active state DEA licensure.
- Master's degree in Nursing (MSN).
- Five or more years of nursing experience; at least two years of nurse practitioner experience preferred.
- Strong interpersonal communication skills – verbal and written.
- Excellent time management skills with accountability for practitioner's daily workflow.
- Proficiency in medical instruments and equipment.
- Strong knowledge base of HIPAA and OSHA regulations.
