Clinical Liaison

We are seeking a skilled Clinical Liaison to join our team at CenterWell Home Health. As a Clinical Liaison, you will play a critical role in developing and maintaining relationships with key hospital and healthcare partners to provide ongoing updates on our services.

• Conduct facility visits to assist our clinical team in determining patient eligibility for our services.
• Ensure effective communication and collaboration with our program staff and other field sales resources.
• Assist in the timely processing of physician orders as directed.

• RN/LPN/LVN license.
• Strong knowledge of governmental regulations and Medicare eligibility requirements.
• Excellent customer service, account development, and communication skills.

• Bachelor's of Science in Nursing.
• A minimum of three years clinical experience.
