Job Title:
Long Term Care Pharmacist

Job Overview:
We are seeking a skilled Long Term Care Pharmacist to join our team at A-Line Staffing Solutions. As a Long Term Care Pharmacist, you will be responsible for maintaining patient/resident databases, ensuring laboratory tests are ordered and evaluated, and collaborating with the healthcare team to provide clinical and technical support.

Responsibilities:
• Maintains patient/resident database to include current and past medical history, clinical and laboratory data, and concurrent medication.
• Ensures laboratory tests for monitoring high-risk conditions are ordered, evaluated, and discussed with the healthcare team.
• Collaborates with the healthcare team to provide clinical and technical support as needed.
• Documents all communication with physicians, nurses, and other members of the healthcare team in the medical record.
• Implements policies and procedures for the safe procurement, storage, distribution, and use of drugs and biologicals.
• Obtains and maintains required documentation in accordance with federal and state regulatory agencies and policy.
• Informs the Pharmacy Manager of medication-related errors.
• Assists facility staff with procedures relating to safe and effective methods of procuring, maintaining, administering, and disposing of drugs.

Qualifications:
• Bachelor's Degree required – PharmD preferred.
• 1+ year of pharmacist experience – LTC preferably.
• Must have Georgia Pharmacist License.
