Job Summary
Aveanna Healthcare is seeking a skilled and compassionate Private Duty Nurse - LVN to join our team. As a Private Duty Nurse, you will provide one-on-one care to our patients in their homes, assisting them with transitioning from the hospital and providing continuous care to remain at home with their loved ones.
Responsibilities
- Implement and manage physicians' prescribed plans of care for our patients
- Conduct patient assessments and coordinate care with our team
- Administer prescribed medication, treatments, and therapies as needed
- Document care provided in our system of record in a timely and compliant manner
- Teach and train family members on health promotion and disease management
- Accept and write verbal orders from physicians as required, and obtain co-signature of Case Manager on all verbal orders
- Follow up with physicians, clients, family members, staff, and Case Manager on client problems, concerns, and/or changes in client condition
Requirements
- Active, unencumbered license (RN/LPN/LVN) in the state in which the clinician will practice
- Current CPR certification from American Red Cross, American Heart Association, or BLS
- TB skin test (current within last 12 months)
- Six months prior hands-on nursing experience preferred but not required
- Reliable transportation
