Job Summary:
We are seeking a skilled and compassionate Private Duty Nurse RN to join our team. As a Private Duty Nurse RN, you will provide one-on-one care to adult patients in their homes, delivering high-quality nursing services and promoting independence and well-being.
Responsibilities:
- Provide skilled nursing care to adult patients in their homes, including wound care, medication management, and vital sign monitoring
- Develop and implement individualized care plans to meet the unique needs of each patient
- Collaborate with patients, families, and healthcare teams to ensure seamless care coordination
- Conduct thorough assessments and evaluations to identify patient needs and develop effective care plans
- Document patient progress and outcomes in a timely and accurate manner
Requirements:
- Current RN licensure in the state of employment
- Minimum 1 year of experience as a Private Duty Nurse RN
- Strong communication and interpersonal skills
- Ability to work independently and as part of a team
- Flexibility to work varied schedules, including evenings and weekends
