About the Role

At Sunrise Senior Living, we are seeking a dedicated and compassionate Care Manager to join our team. As a Care Manager, you will be responsible for providing exceptional care and services to our residents in our assisted living and reminiscence neighborhoods.

Key Responsibilities
  • Build Meaningful Relationships: Develop strong relationships with a specified number of seniors and their families, providing assistance with activities of daily living and attending to individual care needs.
  • Notify Management of Changes: Inform management of any changes in a resident's condition and recommend adjustments in the level of care and service as needed.
  • Assist with Life Skills: Assist residents with life skills and other life-enriching activities as indicated on their individual profile.
  • Participate in ISP Development: Participate in the development of Individualized Service Plans (ISPs) and monthly updates.
  • Ensure Resident Safety: Responsible for their designated group of residents during the shift, knowing where they are and physically checking on them throughout the shift.
  • Partner with Community Team: Collaborate with the community team to ensure compliance with OSHA requirements and promotion of Risk Management programs and policies.
  • Provide Excellent Customer Service: Respond to the dining needs of residents and guests while maintaining Sunrise hospitality and service standards.
Qualifications
  • Availability: Must be available to work one weekend shift.
  • Passion for Serving Seniors: Dedication to and passion for serving seniors with excellent customer service skills.
  • Positive Attitude: A positive attitude, flexibility to perform various duties, and ability to work in a team environment are essential.
  • Education and Certifications: High School diploma/GED accepted and may be required per state regulations. In states where applicable, must maintain certifications.
  • Age Requirement: Must be at least 18 years of age.
  • Previous Experience: Previous experience working with seniors preferred.
  • Decision-Making Skills: Ability to make choices, decisions, and act in the resident's best interest.
  • Communication Skills: Possess written and verbal skills for effective communication and a level of understanding.
  • Organizational Skills: Competent in organizational and time management skills.
  • Problem-Solving Skills: Demonstrate good judgment, problem-solving, and decision-making skills.
