Care Manager

At Sunrise Senior Living, we're passionate about providing exceptional care and services to seniors and their families. As a Care Manager, you'll play a vital role in delivering high-quality care and creating a warm, welcoming environment for our residents.
Responsibilities:- Build meaningful relationships with seniors and their families, providing assistance with daily living activities and attending to individual care needs.
- Notify management of changes in condition and recommend adjustments in care and services.
- Assist residents in life skills and other life-enriching activities, as indicated on their individual profiles.
- Participate in the development of Individualized Service Plans (ISPs) and monthly updates.
- Responsible for a designated group of residents during shifts, ensuring their safety and well-being.
- Partner with the community team to ensure compliance with OSHA requirements and promote Risk Management programs and policies.
- Respond to dining needs of residents and guests, maintaining Sunrise hospitality and service standards.
- Invite, Encourage, and Assist (IEA) residents in life skills and other life-enriching activities, as indicated on their ISP and demographic profiles.
- Must be available weekends.
- Must have a valid HHA certificate.
- Dedication to serving seniors with excellent customer service skills.
- Positive attitude, flexibility, and ability to work in a team environment.
- High School diploma/GED accepted and may be required per state regulations.
- In states where applicable, must maintain certifications.
- Must be at least 18 years of age.
- Previous experience working with seniors preferred.
- Ability to make choices, decisions, and act in the resident's best interest.
- Possess written and verbal skills for effective communication and a level of understanding.
- Competent in organizational and time management skills.
- Demonstrate good judgment, problem-solving, and decision-making skills.
