Job Summary

We are seeking a skilled Licensed Practical Nurse (LPN) to join our team at Junction City Rehabilitation LLC. As an LPN, you will play a vital role in providing high-quality patient care and contributing to the overall success of our healthcare facility.

Key Responsibilities
  • Administer medications and review medication cards for accuracy and completeness.
  • Develop and implement individualized care plans for residents, ensuring their needs are met and exceeded.
  • Collaborate with nursing staff and support personnel to plan and coordinate daily services, programs, and activities.
  • Conduct daily rounds to ensure staff is providing direct care in accordance with resident care plans and wishes.
  • Develop and maintain rapport with nursing staff, residents, family members, and other individuals involved in resident care.
  • Participate in facility surveys and maintain confidentiality of all resident care information.
Requirements
  • Current, unencumbered, active license to practice as an LPN in this state.
  • Nursing degree from an accredited college or university.
  • Knowledge of nursing and medical practices, laws, regulations, and guidelines.
  • Experience with Electronic Medical Records and computer documentation systems.
