We are seeking a highly skilled and compassionate Registered Nurse to join our team at Illumia, a leading provider of palliative care services. As a Palliative Care RN, you will play a critical role in delivering high-quality, patient-centered care to individuals with advanced illnesses.
Key Responsibilities- Conduct in-person and telephonic assessments to identify patient support needs and develop personalized care plans.
- Collaborate with interdisciplinary teams, including physicians, nurse practitioners, and social workers, to ensure comprehensive care.
- Provide education and support to patients and families, addressing disease process, self-care techniques, end-of-life care, medications, nutrition, and dietary needs.
- Communicate effectively with patients, families, and healthcare professionals to ensure seamless care coordination.
- Document timely clinical records, including screening/assessment findings, physician orders, progress notes, and care plans.
- Support care plan development and implementation, including disease-specific changes, decline or change in function, nutrition and/or cognition, inadequately controlled pain or other distressing symptoms, falls, infections or similar events.
- Current RN license to practice in the state.
- 2 years of prior skilled Registered Nurse experience.
- Demonstrated ability to provide quality patient care.
- Nursing experience in palliative care, oncology, hospice, or geriatric care preferred.
