Palliative Care Nurse

We are seeking a highly skilled and compassionate Registered Nurse to join our team as an Afterhours Palliative Care RN. As a key member of our interdisciplinary team, you will play a vital role in providing high-quality patient care and support to individuals and families facing advanced illnesses.
Key Responsibilities- Assess and coordinate patient/family needs, providing preventive and supportive care to palliative patients and their families.
- Utilize assessment, intervention, and teaching skills to maximize patient comfort and enhance the quality of life.
- Assume afterhours coverage for designated teams, functioning as an interdisciplinary advanced illness management and support specialist.
- Triaging patient needs over the phone, making initial nursing evaluations, and collaborating with patients, families, and healthcare teams to provide comprehensive care.
- Documenting patient progress, maintaining accurate clinical notes, and communicating effectively with team members and healthcare providers.
- Current RN license to practice in the state.
- 2 years of prior skilled Registered Nurse experience, preferably in palliative care, oncology, hospice, or geriatric care.
- Demonstrated ability to provide quality patient care, with strong compassionate phone skills and sound critical decision-making.
- Fluent competence in computer programs, including Microsoft Outlook, Word, and Excel, as well as electronic medical record documentation.
