Occupational Therapist for Home Health

FeldCare Connects is a leading referral network seeking a dedicated and compassionate Occupational Therapist to deliver exceptional care to adults of all ages in the Santa Clarita area and surrounding communities.
Key Responsibilities:- Conduct thorough assessments and evaluations to determine patient care needs, focusing on functionality status, activities of daily living, fine motor coordination, and home assessments.
- Develop and implement personalized treatment plans, incorporating adaptive equipment and therapeutic activities to enhance patient wellbeing.
- Collaborate with the healthcare team to ensure seamless care coordination and effective communication.
- Provide education and training to patients, families, caregivers, and healthcare professionals on occupational therapy techniques and strategies.
- Occupational Therapist license and registration in the state
- Completion of an accredited Occupational Therapist program
- Ability to work with adults of all ages and enthusiasm for delivering exceptional care
- Bilingual skills a plus
- Flexibility to create your own schedule and work in your preferred area
- Independence as an independent contractor, with above-average compensation and expense write-offs
