Occupational Therapist for Home Health

We are seeking a highly motivated and compassionate Occupational Therapist to join our team of dedicated healthcare professionals. As an Occupational Therapist with FeldCare Connects, you will have the opportunity to make a meaningful difference in the lives of our patients and their families.
About the RoleAs an Occupational Therapist in Home Health, you will be responsible for assessing and evaluating patient care needs, developing and implementing treatment plans, and providing interventions aimed at improving and enhancing patient wellbeing. You will work closely with our interdisciplinary team to ensure seamless care coordination and excellent patient outcomes.
Key Responsibilities- Provide occupational therapy services to patients in their homes, as defined by state laws governing the practice of occupational therapy
- Develop and implement individualized treatment plans, in accordance with the plan of care and in coordination with other healthcare team members
- Lead patients in the use of therapeutic, creative, and self-care activities to improve functioning and safety
- Observe, record, and report patient response to treatment and changes to patient condition
- Instruct patients, families, caregivers, and healthcare team members in areas of therapy in which they can participate in assisting the patient
To be successful in this role, you will need:
- Ability and enthusiasm in working with all ages
- Occupational Therapist license and registration by the state
- Completion of an accredited Occupational Therapist program
- Bilingual is a plus
