Job Title: Rapid Rehousing Case Manager

Covenant House California is seeking a highly skilled and compassionate Rapid Rehousing Case Manager to join our team. As a key member of our Orange County program, you will provide critical support to young people experiencing homelessness, helping them achieve stable housing and a brighter future.

Job Summary:

The Rapid Rehousing Case Manager will work closely with our young people, aged 18-24, who are experiencing homelessness and facing multiple barriers. Your primary responsibility will be to provide individualized case management services, including:

  • Intake and assessment to identify young people's needs and goals
  • Development of Individual Success Plans (ISPs) to guide their housing navigation and case management
  • Direct assistance with housing applications, lease reviews, and budgeting
  • Regular check-ins and support to ensure young people's progress and well-being
  • Collaboration with community agencies and partners to provide comprehensive services

You will work in a fast-paced environment, prioritizing tasks and managing multiple cases while maintaining a trauma-informed and strength-based approach. Your exceptional communication and organizational skills will enable you to build strong relationships with young people, colleagues, and community partners.

Responsibilities:
  • Provide case management services to young people, including intake, assessment, and ISP development
  • Assist young people with housing applications, lease reviews, and budgeting
  • Conduct regular check-ins and provide support to ensure young people's progress and well-being
  • Collaborate with community agencies and partners to provide comprehensive services
  • Document progress and outcomes in ETO and HMIS
Requirements:
  • Bachelor's degree in Psychology, Sociology, Social Work, or related field (or equivalent experience)
  • Minimum 4 years of case management experience, preferably with young people experiencing homelessness
  • Excellent communication and organizational skills
  • Ability to work in a fast-paced environment and prioritize tasks
  • Knowledge of adolescent development, community resources, and case management skills
