LVN - Part Time Job Description

We are seeking a skilled LVN to join our team at Montare Behavioral Health & Sanctuary Treatment Center. As a LVN, you will play a vital role in providing community substance use nursing services, focusing on prevention, treatment, education, and rehabilitation of the physical and mental health of our clients.

Key Responsibilities:
  • Assist with self-administration of medication policy, documenting and observing patient medication regimens.
  • Ensure accurate documentation of medication orders.
  • Observe patients, charting and reporting changes in patients' conditions, and take necessary action.
  • Measure and record patients' vital signs, such as height, weight, temperature, blood pressure, pulse, and respiration when required.
  • Evaluate medical concerns and direct patients to emergency medical care when necessary.
  • Work as part of a healthcare team to assess patient needs, plan and modify care, and implement interventions.
  • Review medication logs and consult with patients and physicians regarding refills.
  • Provide monitoring and supervision of patients to support non-clinical staff.
  • Request inventory and requisition supplies and instruments.
  • Schedule and transport clients to medical and dental appointments.
  • Facilitate Health and Wellness groups and document clients' participation.
  • Assess new admits for medical necessity and develop a detox treatment plan based on physicians' orders when required.
  • Perform breath analysis and monitor urine test samples from patients, documenting and recording results.
Requirements:
  • Attainment of at least 18 years of age.
  • LPT licensure required.
  • Possession of a valid license/certificate of registration issued by the California Board of Nursing Education and Nurse registration is required.
  • Experience working in a substance use facility preferred.
  • Experience working with clients in acute withdrawal from addictive substances, including COWs and CIWA assessments.
  • Demonstrated ability to work as a team player, with excellent communication skills both oral and written.
  • Valid driver's license, clean driving record, and personal automobile insurance.
Pre-Employment Requirements:
  • Hiring is contingent upon completion of a Background Check, TB test, and clearance on a drug screen and pre-employment physical.
  • Current First Aid and CPR certification or willingness to obtain upon hire.
  • For the mental health program, additional requirements include receiving DOJ/FBI fingerprint clearance or an exemption and a submission of a signed Criminal Record Statement (LIC 508).
