Key Responsibilities- Develop and implement individualized care plans in collaboration with patients, families, and interdisciplinary teams.
- Conduct thorough patient assessments and identify areas for improvement.
- Provide education and support to patients and families throughout the rehabilitation process.
- Collaborate with healthcare teams to ensure seamless care transitions and optimal patient outcomes.
- Participate in quality improvement initiatives and contribute to a culture of excellence.
- Graduation from an accredited Bachelor's of Science in Nursing, Associate Degree in Nursing, or Nursing Diploma program.
- Current state licensure as a Registered Nurse.
- CPR/BCLS certification; ACLS preferred.
- CRRN certification preferred.
- Minimum six months' experience in a medical/surgical setting, preferably in an acute care environment.
- Excellent communication and interpersonal skills.
- A competitive hourly rate of $29-$48.
