As a Home Health Physical Therapist, you will work closely with our multidisciplinary team to develop and implement individualized care plans that promote independence, mobility, and overall well-being.
Responsibilities:- Conduct thorough assessments and evaluations to identify client needs and goals.
- Develop and implement personalized care plans that address physical, emotional, and social needs.
- Provide one-on-one therapy sessions to clients in their homes, focusing on mobility, strength, balance, and safety.
- Collaborate with other healthcare professionals to ensure seamless care coordination and communication.
- Document client progress, goals, and outcomes in a timely and accurate manner.
- Current CO Physical Therapist license.
- Outpatient experience preferred but not required.
- Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association.
- Ability to work independently and manage time effectively.
- Strong interpersonal, organizational, and problem-solving skills.
- Competitive hourly rate starting at $40-$47 per hour, based on experience.
