About the Role

We are seeking a skilled Occupational Therapist to join our team at CenterWell Home Health. As a Home Health Occupational Therapist, you will play a vital role in helping patients manage chronic conditions and recover from injuries or illnesses.

Key Responsibilities
  • Assess and screen patients' daily living and work-related skills, developing therapeutic retraining programs with measurable objectives.
  • Administer and interpret diagnostic and prognostic tests to assist physicians in evaluating patient function.
  • Confer with physicians and other healthcare team members to develop and revise treatment plans.
  • Provide therapeutic treatment and instruction to patients, improving strength, coordination, range-of-motion, and function.
  • Review and expand retraining programs to align with patient goals and rehabilitation program guidelines.
  • Consult with vocational team members to develop vocationally oriented plans.
  • Report patient status and progress to physicians, rehabilitation staff, and family members.
  • Participate in care coordination and discharge planning activities.
  • Design community reintegration activities to assist patients in physical reconditioning and psychological adjustment.
  • Recommend and design special adaptive equipment to improve patient environments.
  • Train staff and family members to carry out retraining programs.
  • Supervise and evaluate Occupational Therapy Assistant and Home Health Aide performance.
Requirements
  • Degree from an accredited Occupational Therapy Program.
  • A minimum of six months of occupational therapy experience preferred.
  • Home Health experience a plus.
  • Current and unrestricted OT licensure.
  • Current CPR certification.
  • Good organizational and communication skills.
  • A valid driver's license, auto insurance, and reliable transportation.
