The Nurse Navigator will play a crucial role in enhancing the patient experience in Breast Imaging by providing navigation services from screening mammograms to diagnostic services and onto the treatment process. This involves providing individualized assistance to patients and families, helping them overcome barriers within and outside the healthcare system.

Key Responsibilities:

  • Coordinate and manage patient care in Breast Imaging through scheduling, supporting, educating, advocating, and providing a link to research.
  • Identify and address barriers to care access, and provide indirect patient care.
  • Monitor and coordinate scheduling, patient tracking, procedure education, and follow-up care to ensure completion of recommended care.
  • Collaborate with patient scheduling, radiologists, social workers, and others to determine and address barriers to care.
  • Coordinate and manage data reporting and tracking in systems such as EPIC, MagView, and other systems to demonstrate reduction of barriers and case management.

Requirements:

  • Bachelor's Degree in Nursing is required.
  • Master's Degree in Nursing is preferred.
  • Three (3) or more years of direct nursing experience and demonstrated clinical competence.
  • Competent in use of Electronic Medical Records.
