Job Opportunity: Home Care Intake Coordinator

VNA Health Group is seeking a skilled professional to fill the role of Home Care Intake Coordinator. As a key member of our home health care team, you will play a vital role in assessing patients' needs and coordinating referrals to home care and hospice.

Responsibilities:
  • Participate in a comprehensive clinical orientation to learn key procedures and the EMR system.
  • Evaluate patients' needs and develop an appropriate plan of care.
  • Provide education and strategies for patients and families.
  • Coordinate referrals to home care to promote continuity of care.
  • Collaborate with other VNA clinicians and external providers to achieve best patient outcomes.
  • Document all changes in patients' conditions and update care plans and outcomes.
Requirements:
  • LPN or RN licensure in New Jersey.
  • 2+ years of home health nursing experience.
  • Excellent verbal and written communication skills.
  • Strong interpersonal and organizational skills.
  • Ability to work in a fast-paced healthcare environment.
