Core Responsibilities:
  • Execute the physician's prescribed care plan.
  • Conduct patient assessments and coordinate care.
  • Administer medications, treatments, and therapies as prescribed.
  • Maintain accurate and timely documentation of care provided.
  • Promote health and educate family members.
  • Receive and document verbal orders from physicians, ensuring co-signature from the Case Manager.
  • Communicate with physicians, clients, family members, and staff regarding patient concerns and changes in condition.
  • Perform additional duties as assigned.
  • Assist patients with transportation to and from school.
Qualifications:
  • Active, unencumbered nursing license (RN/LPN/LVN) in the practicing state.
  • Current CPR certification from recognized organizations.
  • Recent TB skin test (within the last 12 months).
  • Preferred six months of hands-on nursing experience.
  • Reliable transportation is essential.
  • Fingerprinting may be required.
