We are seeking a highly skilled and compassionate Registered Nurse Case Manager to join our team at AccentCare. As a RN Case Manager, you will play a vital role in delivering high-quality patient care and improving health outcomes in the home health setting.
Key Responsibilities- Conduct thorough assessments of patients' medical needs and develop personalized care plans to ensure optimal health outcomes.
- Collaborate with healthcare teams to coordinate patient care and services, including medication management, wound care, and rehabilitation.
- Provide education and support to patients and their families to promote healthy behaviors and self-care.
- Monitor patients' progress and adjust care plans as needed to ensure timely and effective interventions.
- Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination and transitions.
- Graduate from an approved school of professional nursing and currently licensed to practice as a registered nurse in the state of agency operation.
- One (1) year experience as a RN in a home health or similar setting.
- Required Certifications and Licensures: Licensed to practice as a registered nurse in the state of agency operation. Must possess and maintain valid CPR certification while employed in a clinical role. Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order.
- Ability to travel to all business locations.
