Role Overview: In the position of RN Care Manager, you will play a vital role in an Integrated Care Model, communicating essential information regarding member needs and advocating for optimal care solutions. Your efforts will ensure that members receive the appropriate services tailored to their unique requirements.

Field Responsibilities: Expect to allocate approximately 50% of your time visiting members in their residences or long-term care facilities. Flexibility, adaptability, and patience will be crucial as you navigate various situations.

Work Schedule: Our established staff have the option to work four 10-hour days (7 am-6 pm) instead of the conventional five-day, eight-hour schedule, contingent upon demonstrating proficiency in the role and meeting performance metrics.

Key Responsibilities:

  • Evaluate, strategize, and execute personalized care management interventions for each member, focusing on the most suitable and least restrictive care options.
  • Identify and initiate referrals for healthcare and community-based services, including financial, psychosocial, and state-supported resources.
  • Develop and implement care plans throughout the care continuum, serving as a single point of contact.
  • Facilitate communication among all stakeholders to ensure timely and coordinated care and services for all members.
  • Advocate for members and their families to ensure their needs and preferences are represented and supported by the healthcare team.

Performance Recognition: You will be acknowledged for your contributions in a challenging environment that provides clear guidance for success in your role, along with opportunities for professional development.

Required Qualifications:

  • Unrestricted RN license in North Carolina.
  • Resident of North Carolina.
  • A minimum of four years of experience in community health settings or two years in community-based care management for members receiving long-term care or home health services.
  • Intermediate proficiency in Microsoft Office Suite.
  • Willingness to travel up to 50% of the time within designated counties to visit members.

Preferred Qualifications:

  • Bachelor of Science in Nursing (BSN).
  • Certification in Case Management (CCM).
  • Experience managing populations with complex medical or behavioral needs.
  • Experience with Home Care Services.
  • Experience working with members in community and home-based environments.
  • Fluency in Spanish.
