Position Overview:

The Clinical Triage Nurse provides essential telephonic assessment and documentation services that necessitate clinical expertise and interpretation, serving as a vital link between the patient care teams and primary healthcare providers.

Key Responsibilities:

- Assess and prioritize incoming telephone encounters and messages from patients, effectively managing urgent medical triage.
- Facilitate timely evaluations for optimal scheduling into clinician appointments.
- Analyze lab results, imaging outcomes, consultation notes, and clinical summaries, ensuring appropriate referrals to clinicians for further review.
- Maintain clear communication with all healthcare disciplines involved in patient care, offering insights and recommendations to providers.
- Foster professional relationships with healthcare providers and facilities to support home care objectives.
- Collaborate effectively with nursing staff, management, and healthcare providers to enhance patient care delivery.
- Identify individual patient needs for services, referrals, and education, providing necessary guidance on care plans.
- Participate in mandatory meetings and team huddles, contributing to collaborative care efforts.
- Seek resources and guidance as needed in patient care and treatment decisions.
- Offer mentorship and support to non-clinical staff, enhancing team knowledge and skills.
- Coordinate team-based care in the absence of the Program Manager.
- Ensure compliance with federal and local confidentiality regulations, including HIPAA, to protect patient privacy.
- Adhere to organizational policies regarding the safeguarding of patient demographic, clinical, and financial information.
- Perform additional job-related duties as assigned.

Qualifications:

- Must possess a valid Registered Nurse license in the State of Rhode Island or in accordance with the Nurse Licensure Compact agreement.
- Graduate from an accredited nursing program.
- Maintain a current Rhode Island RN license in good standing with 3-5 years of relevant nursing experience.

- Strong collaborative skills to work effectively with colleagues and team members.
- Ability to work independently while demonstrating self-direction.
- Proficient in using technology, including electronic medical record (EMR) systems and Microsoft Office applications.
- Capacity to thrive in a fast-paced and sometimes high-pressure environment.
- Excellent critical thinking, problem-solving, and decision-making abilities.
- Strong organizational skills with the ability to prioritize tasks and follow through on assignments.
- Exceptional verbal and written communication skills.
- Commitment to maintaining quality and safety standards in patient care.

Physical Requirements:

- Primarily remote work with occasional in-office meetings and training sessions.
- Ability to sit or stand for extended periods while using a computer.
- Capability to lift up to 15 pounds and perform manual dexterity tasks.
- Valid driver's license and clear driving record may be required for offsite meetings.
