Role Summary & Key Duties

The Registered Nurse will function as the clinical team leader, reporting directly to the Senior Director of the Virtual Care Center. This role is pivotal in guiding and enhancing patient enrollment, orienting and educating both the care team and patients about Reimagine Care's offerings, and delivering clinical services at designated locations. The ideal candidate will embody Reimagine Care's core values, emphasizing respect and care for patients and colleagues, collaboration, initiative, resourcefulness, and a commitment to advancing our innovation and impact for all stakeholders.

Primary Responsibilities: This position is structured in tiers, with each level building upon the previous one:

Tier 1: Development

  • Establish a constructive working relationship and act as a vital point of contact and clinical resource for patients, healthcare providers, and care teams.
  • Standardize, refine, and enhance workflow processes to improve patient, provider, and care team engagement and satisfaction, optimizing enrollment and overall experience.

Tier 2: Maintenance

  • Analyze patient enrollment data, patient experiences, and provider feedback to identify trends, gaps, and opportunities for enhancement.
  • Collaborate with operational and clinical teams to assess business and clinical needs aligned with established objectives and performance metrics.

Tier 3: Clinical Care

  • Work in partnership with the Virtual Care Center team to deliver patient care while adhering to all Reimagine Care policies, procedures, and best practices for communication and documentation.
  • Comprehend and follow approved symptom management pathways, protocols, and evaluation timelines based on patients' severity levels and care pathways.

Qualifications, Experience, and Skills

Required:

  • Education, Licensure, and Experience:
    • Bachelor of Science in Nursing (BSN)
    • Current and valid Registered Nurse license in the state of practice
    • Minimum of 5 years of clinical experience in oncology
  • Skills:
