Patient Care Navigator RN

Job Summary: We are seeking a skilled Patient Care Navigator RN to join our team at Empath Health. As a key member of our hospice care team, you will be responsible for ensuring the highest level of patient care and comfort.
Key Responsibilities:
- Develop and implement patient care plans in collaboration with the interdisciplinary team.
- Conduct regular assessments to identify patient needs and preferences.
- Provide education and support to patients and their families on end-of-life care and hospice services.
- Maintain accurate and timely documentation of patient interactions and care plans.
- Collaborate with healthcare providers to ensure seamless care transitions.
Requirements:
- Current RN licensure in the state of practice.
- Minimum of 2 years of hospice experience, with a focus on patient care navigation.
- Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
- Ability to work in a fast-paced environment and prioritize multiple tasks and responsibilities.
