Nurse Practitioner HouseCalls Specialist

We are pioneering a new approach to healthcare, one that prioritizes patient-centered care and seamless transitions between care settings. As a member of our HouseCalls team, you will play a vital role in delivering high-quality, in-home assessments that identify and address health risks, improve health outcomes, and enhance the overall well-being of our patients.
Key Responsibilities:- Conduct comprehensive in-home assessments, including evaluations of past medical history, review of systems, medication reconciliation, vital signs, and physical exams.
- Perform evidence-based practice screenings, including point-of-care testing as appropriate.
- Identify diagnoses and develop care management plans to address health risks and improve health outcomes.
- Communicate findings to patients' primary care physicians to inform care and address potential gaps in care.
- Respond to urgent and emergent situations, providing timely interventions as needed.
- Educate patients on disease processes, medication management, and compliance.
- Active NP license or ability to obtain by start date, with unencumbered licensure and no open disciplinary actions.
- Active ANCC or AANP national certification in Family, Geriatrics, Adult, Adult-Gerontology Primary Care, or Adult-Gerontology Acute Care, or ability to obtain certification by start date.
- Ability to complete physical requirements of the job, including lifting, navigating stairs, and sitting, standing, and kneeling for physical assessments.
- Reliable transportation for home visits.
- 1+ years of clinical experience in the highest level of education, clinical setting.
- Experience in gerontology, cardiology, internal medicine, or endocrinology.
- Home health care or home visit experience.
- Ability to travel throughout the state, up to 5% of the time, with advanced notice and potential for additional compensation incentives.
