About the Role

This is a full-time opportunity to work as a provider member within our interdisciplinary team, delivering high-quality home-based medical care to patients in an assigned geography.

Key Responsibilities
  • Establish and maintain effective relationships with patients, caregivers, and healthcare professionals
  • Perform comprehensive assessments, develop patient management care plans, and provide ongoing care
  • Prescribe diagnostics and interventions to avoid unnecessary acute admissions
  • Consult with hospital and emergency teams following patient transfers
  • Implement HEDIS measure campaigns and quality initiatives to ensure high standards of care
Requirements
  • Completion of Nurse Practitioner accredited program
  • Current state RN license and NP license
  • Access to reliable transportation
Preferred Qualifications
  • 2+ years clinical experience in primary care, home-based medical care, geriatrics, or palliative care
  • Knowledge of long-term care, community resources, and cost-effective alternatives
