RN Care Manager

We are seeking a skilled RN Care Manager to join our team at Providence Medical Group. As a key member of our care coordination team, you will play a vital role in providing high-quality, patient-centered care to our most vulnerable patients.
Key Responsibilities- Provide comprehensive care coordination services to patients with complex medical needs, including medication management, wound care, and disease management.
- Collaborate with healthcare providers, family members, and caregivers to develop and implement individualized care plans that promote patient independence and self-management.
- Conduct home visits to assess patient needs, provide education, and support, and make recommendations for care transitions.
- Work closely with our interdisciplinary team to ensure seamless transitions of care and optimal patient outcomes.
- Document patient information, progress, and outcomes in a timely and accurate manner.
- Oregon Registered Nurse License upon hire.
- 2 years Clinical medical-surgical, acute care or long-term care experience that also includes:
- 1 year in community-based/ambulatory care.
- 1 year in care of the frail elderly.
- Bachelor's Degree in Nursing.
- Home health, hospice, palliative care, managed care, or skilled nursing facility background.
- 1 year Care management of diverse patient populations.
