Hospice Care Specialist

We are seeking an experienced Hospice Social Worker to join our team at Endeavor Hospice. As a key member of our interdisciplinary team, you will play a vital role in coordinating patient care and developing comprehensive care plans.
The ideal candidate will have a Master of Social Work (MSW) degree and at least one year of experience in a healthcare setting. You will work closely with physicians and other healthcare professionals to execute patient care plans and maintain compliance with policies, procedures, and regulations.
