POSITION SUMMARY
The primary role of the Clinical Nutrition Specialist is to work collaboratively with the ESRD Interdisciplinary Team to create a personalized treatment strategy for each patient with End-Stage Renal Disease (ESRD), based on a thorough evaluation of their nutritional needs. This position also entails conducting nutrition assessments and formulating dietary plans aimed at enhancing patient health outcomes.

KEY RESPONSIBILITIES
  1. Conduct comprehensive nutritional evaluations for all assigned patients in accordance with established care standards.
  2. Monitor laboratory results, dialysis effectiveness, fluid weight status, and nutritional issues, demonstrating a solid understanding of lab values and their clinical implications.
  3. Engage with patients monthly to review lab findings, dialysis adequacy, and nutrition-related concerns, providing education on necessary dietary adjustments.
  4. Assess and educate dialysis patients regarding their nutritional requirements, including guiding new patients on their prescribed renal diet.
  5. Document monthly nutrition progress in patient medical records.
  6. Evaluate the need for oral supplements or enteral/parenteral nutrition support and facilitate their initiation.
  7. Advise on nutrition-related medications as appropriate and actively participate in monthly Patient Care Conferences.
  8. Collaborate with the ESRD Interdisciplinary Team to formulate individualized treatment plans based on comprehensive patient assessments.
  9. Create and disseminate educational resources on dietary practices for dialysis patients, and conduct informative seminars and classes.
  10. Track nutrition-related Quality Assurance and Performance Improvement (QAPI) data, and engage in QAPI meetings and initiatives.
  11. Provide ongoing education for healthcare staff.
  12. Participate in professional development activities relevant to renal nutrition.
  13. Demonstrate the ability to interact effectively with patients and colleagues from diverse cultural and ethnic backgrounds.
REQUIRED QUALIFICATIONS
  1. Bachelor's Degree in Nutrition Science or a related field.
  2. Current registration with the Commission on Dietetic Registration.
  3. Minimum of one year of clinical experience in nutrition as a registered dietitian is preferred.
WORKING CONDITIONS:
Regular exposure to health risks associated with bloodborne, respiratory, and airborne pathogens. Interaction with patients in various situations, including unpredictable environments. The work setting includes both a quiet office and a busy treatment area.

PHYSICAL DEMANDS:
The physical requirements necessary to successfully perform the essential functions of this role include:
  1. Manual dexterity for writing, typing, and using office equipment.
  2. Prolonged computer use.
  3. Ability to sit for extended periods.
  4. Lifting nutritional supplements weighing up to 20 pounds.
  5. Walking and standing for several hours while engaging with staff and patients.
