Job SummaryWe are seeking an experienced Mental Health Discharge Planner to join our team at the Mesa County Detention Complex in Grand Junction, CO. As a Mental Health Discharge Planner, you will play a critical role in our mission to provide exceptional healthcare services to our patients.
Responsibilities- Conduct rounds with physicians and mental health staff to discuss placement options, organize records for transition, and provide advice on community assistance and transportation options.
- Involve patients in discharge planning and assist them in identifying a support network, instructions for future care, and follow-up.
- Assist patients with completing clinical and financial documentation for social service organizations.
- Coordinate with healthcare and security staff regarding patient release dates.
- Liaison for all referrals for involuntary examinations or crisis stabilization.
- Liaison for all inpatient care management.
- Bachelor's Degree in a human service field.
- Valid CPR card.
- Minimum of 1 year experience in a clinical setting, with correctional healthcare experience preferred.
- Knowledge and experience in developing mental health discharge plans and coordination with community service providers.
