Job Title: Care Manager Assistant

Vaya Health is seeking a highly motivated and detail-oriented Care Manager Assistant to join our team. As a Care Manager Assistant, you will play a vital role in supporting our care managers in delivering high-quality care to our members.

Job Summary:

The Care Manager Assistant will assist care managers in performing various tasks, including member outreach, support, and education. This role requires strong communication and interpersonal skills, as well as the ability to work effectively in a team environment.

Key Responsibilities:
  • Assist care managers in delivering Tailored Care Management services to members
  • Perform member outreach, engagement, and follow-up activities
  • Coordinate services and appointments, including transportation arrangements
  • Engage in health promotion activities and knowledge sharing
  • Provide education on members' chronic conditions and teach self-management skills
  • Share information with care managers and other team members on member circumstances
  • Provide referrals and assistance in obtaining community-based resources and social support services
  • Participate in case conferences and support care managers in assessing and addressing unmet health-related resource needs
Requirements:
  • High school diploma or equivalent required
  • Must be at least 18 years old and trained in Tailored Care Management
  • Must meet one of the following requirements:
    • Be a person with lived experience with an I/DD or a TBI with demonstrated knowledge of and direct personal experience navigating the North Carolina Medicaid delivery system
    • Be a person with lived experience with a behavioral health condition who is a Certified Peer Support Specialist
    • Be a parent or guardian of an individual with an I/DD or a TBI or a behavioral health condition and has at least two years of direct experience providing care for and navigating the Medicaid delivery system on behalf of that individual
    • Have two years of paid experience performing the types of functions described in the Duties/Responsibilities section of this role description in addition to one year of paid experience working directly with the Tailored Care Management eligible population
Preferred Qualifications:
  • Certified PSS (Peer Support Specialist) and/or WRAP (Wellness Recovery Action Plan) Facilitator
Work Environment:

This is a remote position that requires living in or near Wilkes County, North Carolina. The successful candidate will be required to work independently and as part of a team to provide high-quality care to our members.

Salary and Benefits:

Salary will be commensurate with experience and qualifications. This position is non-exempt and is eligible for overtime compensation. Vaya Health offers a comprehensive benefits package, including medical, dental, and vision insurance, as well as a 401(k) plan.
